Use this Record of Books Lent to track:

• Author
• Title
• Name of the person borrowing your book
• Their phone number
• Date the book was uplifted

Tear out a Mrs Blackwell's bookmark and insert it into the book with your name and the date they borrowed your book. This will serve as an ongoing and gentle reminder that you'd like your book back.
